Each report is recorded in a pre-printed form designed by the ICC for use by individual railroad companies. Since not all of the data requested was applicable to the UPT's situation, numerous pages were left blank. The data requested varies from form to form over the years. The 1954 report, for example, includes the following information:

• the identity of the responding party, including the names of members of the UPT Committee and its officers
• balance sheets
• expenditures for road and equipment property
• costs of equipment installed
• insurance policies in force
• special deposits
• debt
• income
• operating expenses
• rail laid
• hours worked by various classes of employee and compensation paid (including overtime)
• compensation of officers
• payments to persons not employed by the Committee
• contracts and agreements in force
